Recommended Dosage for Stiripentol in Dravet Syndrome
Stiripentol (Diacomit) is approved for use with clobazam and valproate in patients aged 2 years and older with Dravet syndrome whose seizures are not adequately controlled with these drugs alone.1
Patients weighing 30 kg or more
- 50 mg/kg/day, given orally in 2 or 3 divided doses.
Patients weighing less than 30 kg
- 50 mg/kg/day, given orally in 2 or 3 divided doses.1
How to Administer Stiripentol
Take with food to improve absorption and reduce side effects like nausea. Powder form must be mixed with water and taken immediately; capsules can be opened and mixed with applesauce if swallowing is difficult.1
Dosage Adjustments
- Start at the full recommended dose; no titration needed.
- Reduce clobazam dose by half when adding stiripentol due to CYP2C19 inhibition, which increases clobazam levels.
- Adjust based on tolerability or weight changes; maximum daily dose is 3,000 mg.
- No specific adjustments for mild-to-moderate hepatic impairment; avoid in severe cases.1
What If a Dose Is Missed
Take as soon as remembered unless close to the next dose. Do not double up.1
Use in Children Under 2 or Other Conditions
Not approved or recommended for children under 2 years. No approvals for other epilepsy types outside Dravet syndrome with the specified regimen.1
Common Dosage-Related Side Effects
Higher doses increase risks of sedation, decreased appetite, and weight loss. Monitor weight and consider dose reduction if needed.1
