Entresto 15/16?

Entresto (sacubitril/valsartan) does not come in a 15/16 mg tablet. The standard tablet strengths are:

- 24 mg sacubitril / 26 mg valsartan
- 49 mg sacubitril / 51 mg valsartan
- 97 mg sacubitril / 103 mg valsartan

If you’re seeing a label or prescription that says 15/16, this isn’t a standard strength. It could be a misprint or a misunderstanding. Please check with your pharmacist or prescriber to confirm the exact dose.

Dosing basics for heart failure with reduced ejection fraction (HFrEF), using the usual strengths:
- Starting dose depends on prior ACE inhibitor/ARB therapy.
- If transitioning from low-dose ACEi/ARB or not on one, start at 24/26 mg twice daily.
- If tolerating and the previous ACEi/ARB dose was moderate/high, titrate to 49/51 mg twice daily, then to 97/103 mg twice daily as tolerated.
- Do not use with ACE inhibitors together; there needs to be an appropriate washout period when switching from an ACE inhibitor to Entresto.
- Maximum recommended is 97/103 mg twice daily.

Important safety notes:
- Do not use in pregnancy.
- Monitor blood pressure, kidney function, and potassium.
- Report signs of angioedema (swelling of face, lips, tongue, throat; trouble breathing).
