Primary Use of Famciclovir
Famciclovir treats herpes zoster (shingles) and genital herpes caused by varicella-zoster virus (VZV) and herpes simplex virus types 1 and 2 (HSV-1, HSV-2). It converts to penciclovir in the body, which inhibits viral DNA polymerase to stop replication.[1]
Can It Treat Other Herpesviruses?
Famciclovir shows activity against other herpesviruses in lab studies and limited clinical use:
- HSV encephalitis or keratitis: Used off-label for severe HSV infections like brain or eye involvement, though acyclovir is preferred.[2]
- VZV in immunocompromised patients: Effective for disseminated VZV beyond shingles.[1]
No strong evidence supports routine use for Epstein-Barr virus (EBV) or cytomegalovirus (CMV); penciclovir has lower potency against CMV.[3]
Activity Against Non-Herpesviruses
Famciclovir lacks efficacy against non-herpesviruses:
- No benefit for hepatitis B or C (HBV, HCV), where nucleoside analogs like entecavir or sofosbuvir are standard.
- Ineffective for influenza, RSV, or HIV; trials showed no antiviral effect.[4]
Penciclovir's mechanism targets herpesvirus thymidine kinase, absent in most other viruses.
Off-Label and Investigational Uses
Limited off-label applications include:
- Recurrent herpes labialis (cold sores).
- Prophylaxis in transplant patients for HSV/VZV reactivation.
Ongoing research explores combinations for resistant HSV strains, but no approvals for other viruses like varicella vaccine-related rash or HHV-6/8.[2][5]
Limitations and Alternatives
Resistance can develop in immunocompromised patients, reducing effectiveness. For non-herpes viruses, alternatives include:
