Standard Adult Dosing for Severe Malaria
Artesunate is primarily used intravenously for severe falciparum malaria in adults. The World Health Organization (WHO) recommends an initial dose of 2.4 mg/kg body weight, followed by 2.4 mg/kg at 12 hours, then 2.4 mg/kg daily for up to 7 days until the patient can switch to oral therapy.[1] For a 60-kg adult, this equals about 144 mg per dose.
Dosing for Uncomplicated Malaria
For uncomplicated cases, artesunate is combined with another antimalarial like mefloquine or amodiaquine in fixed-dose regimens. A common adult dose is 4 mg/kg on day 1 (split into two 2 mg/kg doses 12 hours apart), then 4 mg/kg daily on days 2 and 3. For a 60-kg adult, that's roughly 240 mg on day 1 and 240 mg daily thereafter.[1][2]
Weight-Based Calculation Examples
- 45-65 kg adult: Initial IV dose ~120-150 mg (2.4 mg/kg).
- 65-75 kg adult: ~160-180 mg.
- Over 75 kg: Use actual weight, max initial dose often capped at 180-200 mg in practice.[3]
Adjust for obesity using ideal body weight to avoid overdose.
How Long to Take It
IV artesunate lasts 24-48 hours until oral full-course artemisinin combination therapy (ACT) like artemether-lumefantrine takes over (3 days total). Never use monotherapy beyond 7 days to prevent resistance.[1]
Common Side Effects and Monitoring
Adults report hemoglobin drops (up to 1 g/dL), dizziness, or nausea. Monitor blood smears every 12-24 hours and liver function; post-artesinin delayed hemolysis occurs in 7% of cases.[2][4] Seek medical supervision—self-dosing risks undertreatment or toxicity.
Why Dosing Varies by Region or Brand
WHO guidelines dominate globally, but U.S. FDA approves it (Guilin brand) only for severe malaria under compassionate use, with similar 2.4 mg/kg IV dosing. Rectal formulations (for pre-hospital use) are 10-20 mg/kg single dose.[1][5] No oral monotherapy approved.
