What to ask your insurance provider:
- Whether the drug/service is covered (and the plan/formulary name to reference).
- Your cost before coverage (copay vs coinsurance, deductible status).
- Any prior authorization requirements and expected turnaround time.
- Any step therapy requirements (what you must try first).
- Whether a generic or alternative is covered at a lower cost.
- How “in-network” provider/pharmacy affects coverage.
- Whether there are patient assistance programs or copay cards that apply to your plan.
