Standard Dosing Schedule for Stelara
Stelara (ustekinumab) is injected subcutaneously. Adults with plaque psoriasis, psoriatic arthritis, or Crohn's disease start with an initial dose of 45 mg (or 90 mg if over 100 kg) at week 0, followed by 45 mg (or 90 mg) at week 4, then every 12 weeks.[1]
For ulcerative colitis, the regimen is the same initial doses, then every 8 weeks.[1]
Dosing for Pediatric Patients
Children 6 years and older with plaque psoriasis receive weight-based doses: under 60 kg get 0.75 mg/kg at weeks 0 and 4, then every 12 weeks; 60-100 kg get 45 mg on the same schedule; over 100 kg get 90 mg.[1]
Why the Frequency Differs by Condition
The every-12-weeks interval for most indications reflects Stelara's long half-life of about 21 days, allowing sustained IL-12/IL-23 blockade. Ulcerative colitis uses every 8 weeks due to higher induction needs in that disease.[1][2]
What If a Dose Is Missed
Inject as soon as possible, then resume the original schedule. No double-dosing.[1]
Injection Sites and Self-Administration
Patients self-inject into the thigh, abdomen (avoiding 2 inches around navel), or upper arm (with help). Rotate sites to prevent irritation.[1]
