Cosentyx (secukinumab) dosing for adults with active psoriatic arthritis follows a loading dose followed by maintenance. Patients receive 300 mg subcutaneously at weeks 0, 1, 2, 3, and 4 (total 1500 mg loading). Maintenance is then 300 mg every 4 weeks. This applies regardless of prior anti-TNF use.[1][2]
Dosage for PsA Patients with Co-Existing Conditions
No adjustments for mild to moderate renal or hepatic impairment. For patients over 220 pounds (100 kg), the 300 mg dose remains standard. Pediatric PsA dosing (ages 2+) is weight-based: 75 mg for 15-<50 kg, 150 mg for 50-<100 kg, and 225 mg for ≥100 kg, with similar loading and every-4-week maintenance.[1][3]

Patients switching from TNF inhibitors or IL-17 inhibitors start the full 300 mg loading regimen. No dose reduction or bridging needed; begin at week 0.[1]
