Xgeva bone pain?

Bone pain can happen with Xgeva (denosumab), though it can mean different things depending on timing and severity.

Common / less concerning causes

• Musculoskeletal pain (aching in bones, joints, back, or legs) can occur as a side effect.
• “Bone/ostealgia” may show up after injections, especially if you’ve recently started or had a dose change.

Important causes to watch for

1. Low calcium (hypocalcemia)
   Xgeva can lower calcium, and this can cause pain or muscle symptoms.

   • Call your clinician promptly if you notice: tingling/numbness around the mouth, muscle cramps/spasms, twitching, or spasms.

2. Osteonecrosis of the jaw (ONJ) (not bone pain in general, but jaw pain)

   • Look for: jaw pain, swelling, loose teeth, gum problems, or exposed bone—especially after dental issues.

3. Atypical fractures of the femur (rare, but important)

   • Thigh/groin/hip pain that’s persistent can be an early sign. Get evaluated.

4. Rebound/worsening bone turnover if Xgeva is stopped or delayed

   • Some patients can experience increased bone pain or higher fracture risk if dosing is interrupted.

When to seek urgent help

• Severe pain, rapidly worsening symptoms
• Signs of significant hypocalcemia (muscle spasms, severe tingling, confusion)
• Thigh/groin/hip pain that persists or worsens
• Jaw symptoms with swelling or exposed bone

What you can do now

• Tell your oncologist/doctor about the pain (and when it started relative to the injection).
• Ask whether you need labs—often calcium, magnesium, and vitamin D—and whether you should adjust calcium/vitamin D supplementation (if you’re not already).
• Don’t stop or delay Xgeva without guidance.
