How to administer Repatha
Repatha (evolocumab) is injected subcutaneously once every two weeks or monthly. Use the prefilled SureClick autoinjector or Pushtronex system. Clean the injection site (abdomen, thigh, or upper arm) with alcohol, pinch skin if needed, and inject at a 90-degree angle. Press the button until it clicks; hold for 15 seconds (or follow device instructions). Rotate sites to avoid irritation. Do not shake or reuse needles.[1][2]
Dosage for high cholesterol or cardiovascular risk
Start with 140 mg every two weeks or 420 mg monthly (three 140 mg doses consecutively). For homozygous familial hypercholesterolemia (HoFH), use 420 mg monthly. Take with or without food; no dose adjustments for kidney/liver issues. Doctors may combine it with statins.[1][2]
Preparing for your first dose
Store in refrigerator at 36-46°F (2-8°C); do not freeze. Let warm to room temperature for 30-45 minutes before use. Check expiration and inspect for discoloration. Full training from a healthcare provider is required before self-injecting.[1][2]
What if you miss a dose?
Inject as soon as possible, then resume your schedule. If nearly time for next dose, skip the missed one—do not double up.[1][2]
Common side effects and what patients report
Injection site reactions (redness, pain) affect up to 7% of users. Others include nasopharyngitis, upper respiratory infections, back pain, and flu-like symptoms. Allergic reactions are rare but seek immediate help for rash, swelling, or breathing issues. No increased risk of diabetes or cognitive issues in trials.[1][2]
Who should not use Repatha
Avoid if allergic to evolocumab or its ingredients. Pregnancy safety unknown (use in pregnancy registry); not recommended during breastfeeding. Inform doctor of immune conditions or prior hypersensitivity.[1][2]
How Repatha compares to other cholesterol drugs
Repatha, a PCSK9 inhibitor, lowers LDL cholesterol by 50-70% when added to statins, outperforming ezetimibe alone. Unlike statins, it targets a different pathway with fewer muscle side effects but requires injections.[1]
Storage and travel tips
Refrigerate; stable at room temperature up to 30 days or 77°F (25°C) for shorter periods. Discard if expired or damaged. For travel, use cooler packs.[1][2]
