Can you take ramipril with Advil (ibuprofen)?
Ramipril (an ACE inhibitor) can interact with Advil (ibuprofen). Using them together can increase the risk of kidney problems, especially in people who are older, dehydrated, have chronic kidney disease, heart failure, or are taking diuretics (“water pills”). [1]
What’s the main risk?
The combination can reduce blood flow to the kidneys (ibuprofen can constrict kidney blood vessels), while ramipril can lower kidney filtration pressure. Together, this can raise the chance of acute kidney injury and can also worsen potassium control in some patients. [1][2]
Does it increase blood pressure or heart risk?
Ibuprofen can also reduce the blood-pressure–lowering effect of ramipril in some people. [1]
Is the risk only with long-term Advil use?
Risk is higher with higher doses, repeated/longer courses of ibuprofen, dehydration, or underlying kidney issues. Occasional single doses may still carry risk, but the concern is strongest when ibuprofen is used more than briefly or in higher amounts. [1][2]
What should patients do instead?
For pain or fever, people taking ramipril are often advised to discuss alternatives such as acetaminophen (paracetamol) with a clinician/pharmacist, since it doesn’t work the same way on the kidneys as NSAIDs. [1]
When to get medical help
Seek medical advice urgently if you notice reduced urination, swelling, sudden weight gain, severe dizziness, or unusual fatigue after starting or increasing ibuprofen while on ramipril. These can be signs of kidney strain or other complications. [1][2]
Practical safety tips if a clinician approves NSAID use
- Use the lowest effective ibuprofen dose for the shortest time.
- Avoid dehydration (drink fluids as appropriate for you).
- Check with a clinician/pharmacist if you take a diuretic, have kidney disease, or are elderly. [1][2]
