How does Lipitor lower LDL cholesterol? Lipitor (atorvastatin) belongs to a class of drugs called statins. It works primarily by blocking an enzyme in the liver called HMG-CoA reductase, which is responsible for making cholesterol. By reducing internal production, Lipitor forces the liver to pull more LDL cholesterol from the blood, lowering levels by 20 to 55 percent depending on dose. It also raises HDL and reduces trigylcerides modestly. Data from clinical trials show consistent results in patients with risk factors or existing cardiovascular disease. What happens when patients combine Lipitor with fish oil? Many patients take both products together. Fish oil supplements deliver omega-3 fatty acids that mainly lower triglycerides by 20 to 30 percent at doses of 2 to 4 grams per day. They do not lower LDL cholesterol and may raise it slightly in some people. When added to a statin like Lipitor, fish oil provides complementary rather than competing effects. Clinical studies show the combination does not interfere with the statin’s LDL-lowering action. Why do some patients prefer fish oil over Lipitor? Patients who cannot tolerate statins due to muscle pain or liver enzyme changes seek alternatives. Fish oil is sold over the counter and does not require a prescription. It has milder side effects and is often viewed as a natural option. However, the FDA has approved only certain prescription omega-3 products for very high triglyceride levels. Over-the-counter supplements vary widely in purity and content. When does the Lipitor patent expire and are biosimilars available? Lipitor’s patent expired years ago. Generic atorvastatin is now widely available at low cost. Prescription fish oil products such as Lovaza and Vascepa also have generics entering the market. No biological products or biosimilars are involved because both substances are small-molecule drugs or supplements.
