What causes tigecycline diarrhea and why does it occur? Tigecycline disrupts normal gut bacteria, allowing Clostridioides difficile (C. diff) to overgrow and produce toxins that trigger colitis and watery diarrhea. This is a known risk with nearly every broad-spectrum antibiotic, especially those wie tetracycline class. What should you do first when diarrhea develops? Stop taking tigecycline immediately unless stopping it is medially dangerous. Contact your doctor right away for evaluation. Doctors typically test stool for C. diff toxin or glutamate dehydrogenase plus toxin EIA test. If positive, tigecycline is discontinued and treatment switches to vancomycin or fidaxomicin orally. How does C. diff colitis from tigecycline get treated? First-line treatment for non-severe C. diff infection is oral vancomycin 125 mg four times daily for 10 days. Fidaxomicin 200 mg twice daily for 10 days is an alternative that shows lower recurrence rates. Severe cases need intravenous metronidazole or oral metonidazole plus oral vancomycin. The patient must be isolated to prevent spread. What happens if the C. diff infection recurs? Recurrences are common after tigecycline-associated C. diff infections. For first recurrence, repeat oral vancomycin or use fidaxomicin. For multiple recurrences, fecal microbiota transplantation is considered after two or more failures of standard therapy. Patients also receive bezlotoxumab, a monoclonal antibody that prevents toxin binding, with the erkrankungswat and erkrankungswat.
