Does Medicare cover Pennsaid (diclofenac topical)?
Pennsaid is a brand of diclofenac topical solution/gel. Medicare coverage depends on (1) whether you’re using Pennsaid through a prescription plan and (2) which benefit you have.
If Pennsaid is covered, it’s usually under Part D (prescription drug) rules
Medicare Part B generally covers injected or administered therapies and certain outpatient medical services, not most prescription topical medications bought at a pharmacy. Prescription drugs like Pennsaid are typically covered under Medicare Part D, if they’re on your plan’s formulary and you meet plan rules (prior authorization, step therapy, or quantity limits).
If you have Original Medicare + Part D
You’ll generally need:
- A prescription for Pennsaid from your clinician
- Your Part D plan to list Pennsaid (or its generic diclofenac topical equivalent) on the formulary
- Payment of your Part D costs (copay/coinsurance/deductible), which vary by plan
If you have a Medicare Advantage plan
Many Medicare Advantage plans also use a prescription drug benefit (often similar to Part D). Coverage will depend on that plan’s formulary and rules.
What if Pennsaid isn’t on your plan’s formulary?
If your plan doesn’t cover Pennsaid, coverage may still be possible if your clinician requests an exception or prior authorization. Common plan pathways include:
- Switching to a covered diclofenac topical product (such as a different brand strength or the generic)
- Step therapy (using a preferred alternative first)
- Prior authorization for medical necessity
How to check your specific coverage quickly
The fastest way to confirm whether Pennsaid is covered is to:
- Look up Pennsaid on your plan’s drug formulary (or the plan’s app/website)
- Search by the active ingredient (diclofenac topical) and the strength/formulation
- Ask the pharmacist to run it through your Part D/Advantage coverage (they can tell you whether it’s covered and what you’d pay)
If you share your plan type (Original Medicare + Part D vs Medicare Advantage) and the Pennsaid strength/formulation, I can help you figure out what to look for in the formulary rules.
