Vascepa's Main Effect on Lipids
Vascepa (icosapent ethyl) is a purified EPA omega-3 fatty acid approved to lower triglycerides, not cholesterol directly. It reduces postprandial (after-meal) triglyceride spikes from high-fat meals by about 20-45% in clinical studies, without significantly altering total cholesterol or LDL-C levels.1[2]
Mechanism After High-Fat Meals
High-fat meals trigger chylomicron production in the gut, which carry dietary triglycerides into blood. Vascepa works by:
- Enhancing clearance of these chylomicron remnants via lipoprotein lipase (LPL) activation and increased hepatic uptake.
- Reducing hepatic VLDL-triglyceride secretion, limiting the pool of triglycerides available for postprandial surges.
- Inhibiting apoC-III, a protein that blocks LPL, speeding triglyceride breakdown.[3][4]
This selectively targets triglycerides (TG), dropping postprandial TG peaks within 4-6 hours after fatty meals, as seen in trials like MARINE and ANCHOR.1
Does It Lower Cholesterol?
No direct cholesterol reduction. Postprandial studies show:
- No change or slight LDL-C increase (up to 5-10%, due to TG-lowering shifting particles).
- HDL-C stable or modestly up.
- Non-HDL-C and apoB often decrease indirectly via remnant clearance.[2][5]

Compared to Other Omega-3s
Vascepa (pure EPA) outperforms mixed EPA/DHA fish oils (e.g., Lovaza), which raise LDL-C more after fatty meals due to DHA's effects on VLDL.[4][9] Prescription strength (4g/day) amplifies postprandial benefits over OTC supplements.
Patient Considerations and Timing
