How to Use the Ozempic Pen Step by Step
Ozempic (semaglutide) comes in a prefilled, disposable pen for weekly subcutaneous injections. Always follow your doctor's instructions and the patient information leaflet. Pens are single-patient use only—do not share.
1. Prepare the pen: Wash hands. Check the pen label for your dose (0.25 mg, 0.5 mg, 1 mg, or 2 mg). Remove the pen cap. Inspect the solution—it should be clear and colorless. Do not use if cloudy, discolored, or expired.
2. Attach needle: Peel off paper tab from a new needle (included or NovoFine Plus). Screw needle straight onto pen until secure—do not overtighten. Remove outer and inner needle caps; save outer cap.
3. Prime the pen (air check): Turn dose selector to flow check symbol (looks like a droplet). Hold pen upright, tap cartridge to settle air bubbles. Press button fully until 0 shows and a drop appears at needle tip. Repeat up to 6 times if no drop.
4. Select dose: Turn dose selector to prescribed dose. If it won't turn, dose is too high for remaining medicine—contact doctor or pharmacist.
5. Inject:
- Choose site: Abdomen (at least 2 inches from navel), thigh (front), or upper arm. Clean with alcohol swab, let dry.
- Insert needle at 90-degree angle (or 45 if thin).
- Press button fully and hold for 6 seconds—count slowly. Needle may move slightly.
- Release skin. If blood appears, press cotton ball; do not rub.
6. Finish: Unscrew needle, discard in sharps container. Replace pen cap. Store unused pens in fridge (36-46°F/2-8°C) or room temp (up to 56 days, below 86°F/30°C). Do not freeze.
Inject once weekly, same day each week, with or without food. Start at 0.25 mg for 4 weeks, then increase as prescribed.
Where to Inject Ozempic
Rotate sites each week: abdomen, thigh, upper arm. Avoid scars, bruises, or navel area. Inject at least 1 inch from last site. No need to pinch skin unless very thin.
Common Mistakes and Fixes
- No prime drop? Repeat priming—up to 6 times max.
- Dial won't turn? Pen is empty or blocked.
- Painful injection? Use room-temp pen (warm in hands), new needle, correct angle.
- Missed dose? Take within 5 days; skip if later and resume schedule.

What If I Mess Up the Dose
