Why aspirin can cause stomach pain
Aspirin irritates the stomach lining and can increase the risk of stomach inflammation and bleeding. That irritation is a common reason people feel stomach pain after taking it, especially when they take doses on an empty stomach or take more than directed.
Aspirin can also worsen symptoms if you already have stomach conditions such as gastritis or ulcers.
What kind of stomach pain is more concerning
Stomach discomfort after aspirin can be mild, but seek urgent medical care if pain comes with warning signs of bleeding, such as:
- Black, tarry stools
- Vomiting blood or material that looks like coffee grounds
- Severe or worsening abdominal pain
- Dizziness, fainting, or weakness
These symptoms can signal gastrointestinal bleeding, which aspirin can increase.
What to do if aspirin is already causing pain
Stop taking aspirin and contact a clinician for advice, especially if the pain is persistent or getting worse. If a clinician recommends an alternative pain reliever, choose one based on your medical history (for example, whether you have ulcer/bleeding risk).
Avoid taking more aspirin “to push through” the pain. Also avoid alcohol and other medicines that can irritate the stomach unless your clinician tells you it is safe.
Can enteric-coated aspirin help?
Some “enteric-coated” aspirin tablets are designed to dissolve later in the digestive tract, but they do not eliminate the risk of stomach irritation and bleeding for many people. If aspirin is already causing pain, switching formulations may not solve the problem.
What to use instead for pain (common options)
For many people, acetaminophen (paracetamol) may be easier on the stomach than aspirin, but it still must be used carefully based on your health and other medications (especially liver disease and heavy alcohol use).
If you take aspirin for heart protection, do not stop it without medical guidance.
When to get medical help quickly
Get urgent care now (or emergency help) if you have:
- Black/tarry stools, vomiting blood, or severe abdominal pain
- Trouble breathing, swelling of the face/lips, or widespread hives (allergy signs)
- Symptoms of severe weakness or fainting
