Standard Adult Dosage for Hypertension
Losartan, sold as Cozaar, starts at 50 mg once daily for high blood pressure in adults. Doctors often increase to 100 mg once daily if needed, based on blood pressure response. The maximum is 100 mg per day, taken with or without food.[1]
Dosage for Children with Hypertension
For kids 6 years and older weighing at least 20 kg (about 44 lbs), the starting dose is 0.7 mg/kg up to 50 mg once daily. It can go up to 1.4 mg/kg or 100 mg daily, whichever is lower. Not recommended under 6 years.[1]
Dosage Adjustments for Kidney or Liver Issues
In patients with mild to moderate liver impairment, start at 25 mg once daily. No specific adjustment for mild kidney issues, but use caution in severe cases or dialysis. Elderly patients follow standard adult dosing.[1]
How Losartan Fits with Other Blood Pressure Meds
Often combined with hydrochlorothiazide (as Hyzaar) at 50 mg/12.5 mg or 100 mg/25 mg once daily. For heart failure, it's 25-50 mg daily, sometimes up to 150 mg split into two doses, though not FDA-approved for this.[1]
When to Take It and Missed Doses
Take at the same time daily. If missed, take as soon as remembered unless close to next dose—skip and resume schedule. Don't double up.[1]
