Does Losartan Protect or Harm Kidneys?
Losartan, an angiotensin II receptor blocker (ARB), lowers blood pressure by relaxing blood vessels. It protects kidneys in patients with type 2 diabetes and nephropathy by reducing proteinuria and slowing kidney disease progression, as shown in the RENAAL trial where it cut end-stage renal disease risk by 28% compared to placebo.[1] In chronic kidney disease (CKD), it slows glomerular filtration rate decline when used with other therapies.[2]
How Does Losartan Work on Kidneys?
Losartan blocks angiotensin II at AT1 receptors in the kidneys, reducing intraglomerular pressure and hyperfiltration damage. This preserves kidney function better than some alternatives in high-risk groups, like those with proteinuria.[3]
When Can Losartan Harm Kidneys?
Losartan can cause acute kidney injury (AKI) in dehydrated patients, those with bilateral renal artery stenosis, or severe heart failure due to reduced renal perfusion. Risk rises with NSAIDs, diuretics, or ACE inhibitors in combination. Monitor serum creatinine and potassium; hyperkalemia occurs in 1-2% of users.[4][5]
Who Should Avoid Losartan for Kidney Reasons?
Avoid in pregnancy (fetal kidney toxicity), solitary kidney with artery stenosis, or advanced CKD (eGFR <30 mL/min) without specialist oversight. Black patients may see less blood pressure benefit but similar renal protection.[6]
Monitoring Kidneys on Losartan
Check kidney function (eGFR, creatinine) and electrolytes within 1-2 weeks of starting or dose changes, then periodically. Dose adjustments needed below eGFR 30 mL/min.[7]
