Paxlovid blocks the CYP3A4 enzyme that breaks down many statins, so blood levels of those statins rise and stay higher longer. The interaction is strongest with simvastatin and lovastatin, which the FDA warns against using together with Paxlovid because of the risk of muscle damage and kidney injury.

Most other statins are also affected. Atorvastatin and rosuvastatin can be continued, but doctors usually lower the dose or pause the statin for the five days of Paxlovid treatment.

Why does the interaction happen?

Ritonavir, one of the two drugs in Paxlovid, is a potent CYP3A4 inhibitor. When the enzyme is blocked, statins that rely on it for clearance accumulate. The result can be myopathy or, in severe cases, rhabdomyolysis.

How long should patients avoid or adjust statins?

The interaction risk lasts only while Paxlovid is being taken and for a short time afterward. Most clinicians restart or return to the usual statin dose once Paxlovid is finished.
