Lipitor (atorvastatin) belongs to the statin class of drugs and carries a pregnancy category X rating. This means animal studies have shown fetal harm, and the drug is contraindicated during pregnancy. FDA reports note developmental toxicity in rats and rabbits at doses near or above human equivalents, but human data remain limited.
Atorvastatin inhibits HMG-CoA reductase, the key enzyme in cholesterol synthesis. Cholesterol is essential for cell membrane formation and fetal growth. Blocking this pathway in animals produces skeletal malformations, gastroschisis, and reduced fetal weight. Animal reports include skeletal abnormalities at 20 times the human dose and gastroschisis at 22 times the human dose.
Human studies are sparse. A small retrospective review of 44 pregnancies exposed to statins found 2 cases of major malformations (5%)—one hydrocephalus and one polydactyly.
