What Pregnancy Categories Mean
The FDA uses categories A through X based on animal and human data:
- Category B means no evidence of risk in animal studies, but no well-controlled human studies exist. Drugs like famciclovir fit here when animal reproduction studies show no fetal harm.[2]
Evidence from Animal Studies
Studies in rats and rabbits at doses up to 4 times the human dose showed no teratogenic effects or fetal loss. Famciclovir converts to penciclovir, the active form, with similar safety in animal models.[1][3]
Human Data and Recommendations
Limited human pregnancy data exists; it's mainly used for herpes zoster or genital herpes. The CDC recommends it for outbreaks in pregnancy if benefits outweigh risks, especially in third trimester for neonatal herpes prevention. No increased birth defects reported in small cohorts.[4]
Risks and Alternatives
Potential risks include theoretical antiviral effects on fetal DNA, but no confirmed issues. Acyclovir (Category B) is preferred for herpes in pregnancy due to more human data; valacyclovir (also B) is another option.[4][5]
Current FDA Labeling Status
