Standard Orencia Infusion Dosing by Weight
Orencia (abatacept) for intravenous infusion in adults with moderate to severe rheumatoid arthritis uses weight-based dosing:
- Less than 60 kg: 500 mg
- 60 to 100 kg: 1,000 mg
- More than 100 kg: 1,500 mg
Administer over 30 minutes at weeks 0, 2, and 4, then every 4 weeks.[1]
How Dosing Works for Rheumatoid Arthritis
This applies to RA patients who have had an inadequate response to TNF antagonists. Infusions are given in a healthcare setting. Premedication isn't required, but monitor for infusion reactions.[1]
Dosing for Polyarticular Juvenile Idiopathic Arthritis
For patients 2 years and older (weight ≥10 kg):
- 10 to <25 kg: 250 mg
- 25 to <50 kg: 500 mg
- ≥50 kg: 1,000 mg
Same schedule as adults: weeks 0, 2, 4, then every 4 weeks. Not studied below 2 years.[1]
Differences from Subcutaneous Orencia
Subcutaneous dosing is fixed at 125 mg weekly after loading doses, regardless of weight, for adults and patients ≥50 kg. Infusions are preferred initially for some patients.[1]
Adjustments for Kidney or Liver Issues
No dose changes needed for mild to moderate kidney impairment or Child-Pugh A/B liver issues. Data limited for severe cases—use caution.[1]
Common Patient Concerns on Infusion Dosing
Patients often ask about weight cutoffs; use actual body weight at first infusion, without ideal body weight adjustments. Overweight patients may get the higher dose to maintain efficacy.[1]
