Recommended Starting Dose for Farxiga
Farxiga (dapagliflozin) is typically started at 10 mg taken orally once daily for adults with type 2 diabetes, in combination with diet and exercise.[1] For heart failure or chronic kidney disease, the starting dose is also 10 mg once daily.[1]
Dose Adjustments for Kidney Function
Reduce to 5 mg once daily if eGFR is below 45 mL/min/1.73 m² at treatment start. Do not initiate if eGFR is less than 25 mL/min/1.73 m² due to limited efficacy.[1]
How to Take It
Take in the morning, with or without food. Swallow whole; do not split, crush, or chew.[1] Missing a dose? Take as soon as remembered unless close to the next one—skip and resume schedule.
Dosing for Specific Conditions
- Type 2 Diabetes: 10 mg once daily; 5 mg if volume depletion risk or on insulin/diuretics.
- Heart Failure (HFrEF, HFpEF): 10 mg once daily, regardless of diabetes status.
- CKD: 10 mg once daily to reduce kidney function decline, CV death, or hospitalization.[1]
Not approved for type 1 diabetes or glycemic control in advanced CKD.
What Happens with Overdose or Missed Doses
Overdose raises dehydration or hypotension risk—seek medical help. No specific antidote; manage symptoms like hemodialysis if needed.[1]
Common Patient Questions on Switching or Stopping
Switching from other SGLT2 inhibitors? Start at 10 mg without washout. Stopping abruptly? Monitor for volume status; taper not required but consult doctor for rebound risks like hyperglycemia.[1]
