How Often Is Orencia Administered?
Orencia (abatacept) dosing depends on the condition treated—rheumatoid arthritis (RA), psoriatic arthritis (PsA), juvenile idiopathic arthritis (JIA), or prophylaxis of acute graft-versus-host disease (aGVHD)—and the administration method: subcutaneous injection or intravenous (IV) infusion.[1]
Subcutaneous Injection Schedule (Most Common for RA, PsA, JIA)
- Loading dose: 125 mg on days 0, 14, and 30.
- Maintenance: 125 mg once weekly thereafter, starting one week after the third loading dose.
- Patients self-administer via prefilled syringe or autoinjector after training.[1]
IV Infusion Schedule (Used for RA, JIA, aGVHD)
- RA and JIA loading: Weight-based dose (500 mg for <60 kg, 750 mg for 60-100 kg, 1,000 mg for >100 kg) on days 0, 14, and 30.
- RA and JIA maintenance: Same weight-based dose every 4 weeks, starting week 8.
- aGVHD prophylaxis: 10 mg/kg (max 1,000 mg) on days 0 (pre-transplant), +1, +3, +7, +14, then every 4 weeks until day 180 post-transplant.[1]
What If a Dose Is Missed?
Take the subcutaneous dose as soon as possible within 3 days, then resume weekly schedule. For IV, contact a doctor; do not double dose.[1]
Differences by Patient Weight or Age
- Adult RA/PsA: Fixed 125 mg SC weekly or weight-based IV.
- JIA (2-17 years): 125 mg SC for ≥50 kg; 87.5 mg for 25-50 kg; 50 mg for 10-25 kg; 4 mg/kg IV for <75 kg.
- Weight changes >20% may require IV dose adjustment.[1]
How Orencia Compares to Similar Drugs
Unlike weekly SC methotrexate or every-2-weeks Humira (adalimumab), Orencia's weekly SC aligns with Enbrel (etanercept) but offers dual IV/SC options. TNF inhibitors like Remicade require infusions every 8 weeks after loading.[1][2]
