The FDA label requires regular ECG monitoring at baseline, day 14 of cycle 1, and at the beginning of subsequent cycles.

Why do patients monitor ECGs so closely?

The drug inhibits CDK4/6 enzymes, but it also blocks potassium channels involved in cardiac repolarization. Blocking these channels slows the return of membrane potential to baseline, thereby lengthening the QT interval. Patients who already have congenital long QT syndrome, take other QT-prolonging drugs, or have electrolyte imbalances are at higher risk.
