Who Should Not Take Vascepa?
Vascepa (icosapent ethyl) is contraindicated in patients with known hypersensitivity to icosapent ethyl or any component of the capsule. This includes severe allergic reactions like anaphylaxis.[1]
When Is It Unsafe During Pregnancy or Breastfeeding?
Avoid Vascepa in pregnancy due to potential fetal harm—animal studies show embryocidal and teratogenic effects at doses similar to human levels. Use only if benefits outweigh risks, and enroll in the pregnancy exposure registry (1-877-311-8972). It is unknown if Vascepa passes into breast milk; weigh risks before nursing.1
What About Severe Liver or Kidney Problems?
Do not use in patients with severe hepatic impairment (Child-Pugh C), as safety and efficacy are unestablished. No specific contraindication for severe kidney disease, but monitor closely, especially with other lipid-altering drugs.[1]
Why Avoid It with Certain Bleeding Risks?
Patients with active pathological bleeding or hemorrhagic stroke should not take Vascepa, due to increased bleeding risk from its antithrombotic effects. Use caution with anticoagulants (e.g., warfarin) or antiplatelets (e.g., aspirin), as it may prolong bleeding time.1
Other Key Warnings Limiting Use
- Diabetes with retinopathy: Higher risk of hemorrhagic events.
- Alcohol use: Increases triglycerides and bleeding risk.
- Pending surgery: Stop Vascepa 7 days before to reduce bleeding.
Avoid in those unable to follow a low-fat diet or with unresolved pancreatitis.[1]
