How Aristada is Administered for Schizophrenia
Aristada (aripiprazole lauroxil) is a long-acting injectable antipsychotic given intramuscularly to treat schizophrenia in adults. It comes in pre-filled syringes of 441 mg, 662 mg, 882 mg, or 1064 mg strengths, administered by a healthcare professional only—never self-administered.[1]
Dosing starts with an initial injection of 662 mg, alongside an oral aripiprazole 15 mg dose on day 1 to ensure quick therapeutic levels. Maintenance follows every 4 or 6 weeks:
- Every 4 weeks: 882 mg
- Every 6 weeks: 1064 mg
Injection sites rotate between the gluteal or deltoid muscle. Gluteal uses a 2-inch needle; deltoid a 1-inch needle. Refrigerate syringes but warm to room temperature for 30 minutes before use. Administer immediately after drawing into the syringe.[1][2]
Why the Oral Starter Dose?
The oral aripiprazole bridges the gap until Aristada converts to active aripiprazole in the body, which takes 20-24 days. Skipping it risks delayed efficacy. An optional 30 mg oral dose on day 8 supports this.[1]
What if a Dose is Missed?
- Less than 3 weeks late (4-week schedule) or 4 weeks late (6-week schedule): Give next dose as soon as possible, resume original schedule.
- More than 3 weeks late (4-week) or 4 weeks late (6-week): Restart with 662 mg initiation dose plus oral aripiprazole.
No more than 7 weeks between doses.[1][2]

Both are aripiprazole extended-release injections for schizophrenia, but Aristada is a prodrug needing enzymatic conversion, allowing higher doses (up to 1064 mg) and 6-week intervals. Maintena maxes at 400 mg every 4 weeks, with direct absorption.[2]
Patient Concerns During Administration
Pain, swelling, or redness at the site affects up to 5% of patients. Rotate sites to minimize. No oral antipsychotics needed after initiation unless switching therapies.[1]
