Does Kevzara Help Rheumatoid Arthritis?
Kevzara (sarilumab) is FDA-approved to treat moderate to severe rheumatoid arthritis (RA) in adults who have not responded well to one or more tumor necrosis factor (TNF) inhibitors.[1] Clinical trials show it reduces RA symptoms like joint pain, swelling, and fatigue by blocking interleukin-6 (IL-6), a protein that drives inflammation.[2]
In the MOBILITY trial, 58% of patients on Kevzara plus methotrexate achieved at least 20% symptom improvement (ACR20 response) at 24 weeks, compared to 44% on placebo plus methotrexate.[3] The TARGET trial confirmed benefits in TNF non-responders, with ACR20 rates of 61% versus 40% on placebo.[3]
How Quickly Does Kevzara Start Working?
Patients often notice reduced pain and stiffness within 2-4 weeks, with peak effects by 12-24 weeks. Full benefits may take 3-6 months, and doctors monitor with blood tests for inflammation markers like CRP.[1][4]
Who Is Kevzara For?
It's prescribed after TNF blockers (like Humira or Enbrel) fail. It's not first-line; methotrexate or other DMARDs come first. Approved for ages 18+, including those with heart disease risks if monitored.[1]
Kevzara vs. Other RA Drugs
| Drug | Mechanism | ACR20 Response (TNF non-responders) | Common Use Case |
|------|-----------|-------------------------------------|-----------------|
| Kevzara | IL-6 inhibitor | 61% at 24 weeks[3] | Post-TNF failure |
| Actemra (tocilizumab) | IL-6 inhibitor | 50-60%[5] | Similar to Kevzara |
| Xeljanz (tofacitinib) | JAK inhibitor | 55-65%[6] | Oral option |
| Rituxan (rituximab) | B-cell depleter | 50%[7] | Refractory RA |
Kevzara matches or exceeds peers in head-to-head data against adalimumab, with better ACR50 responses (35% vs. 20%).[8]
Common Side Effects and Risks
Upper respiratory infections (26%), injection site reactions (8%), and elevated liver enzymes affect 5-10% of users. Black box warning for serious infections, tears in stomach/intestine perforations, and cancer risk. Not for active infections or live vaccines.[1][4] Neutropenia occurs in 10-20%, requiring blood monitoring every 4-8 weeks initially.
What If Kevzara Doesn't Work?
Switch to another class like JAK inhibitors or abatacept. About 30-40% need changes within a year due to lack of response or side effects.[9]
Cost and Access
A 24-week course costs $40,000-$50,000 without insurance. Patient assistance programs from Sanofi/Regeneron cover copays for eligible patients.[10] Biosimilars are not yet available.
