What is Nubain’s (nalbuphine) mechanism of action?
Nubain (nalbuphine) is an opioid analgesic with mixed activity at opioid receptors. It acts as a kappa (κ) opioid receptor agonist and a mu (μ) opioid receptor antagonist/partial agonist. This receptor profile is why it can provide pain relief while having a different side-effect and dependence/withdrawal profile than traditional pure mu agonists like morphine.
How do kappa agonism and mu antagonism change its effects?
Because nalbuphine strongly activates kappa receptors, it produces analgesia and can cause opioid-like effects such as sedation. Its mu antagonism is associated with a ceiling effect for some effects (like respiratory depression and euphoria) and can reduce or reverse mu-mediated effects when taken in the presence of a mu full agonist.
What does that mean for breathing and overdose risk?
For some patients, nalbuphine’s mu antagonism contributes to a ceiling on respiratory depression compared with full mu agonists.
